About the role

Mercy Community Services are on the lookout for a
Social Worker
on a
Permanent Full- Time basis
to join our team as soon as possible. Your role will be based in
Rockhampton
and an excited growing team helping our community.

*The role of the Social Worker is to deliver social work services to consumers in the community under the direct and indirect supervision of the Allied Health Clinical Lead. Working with clients in the community, you will be delivering sound social work services including assessments, investigations and treatment planning. You will join an excited, growing team in Rockhampton and expanding in Yeppoon. Develop your career path with Mercy Community Services.*

Your role involves but not limited to:

  • Provide excellent, evidence based social work interventions to our clients and their families or carers including comprehensive/psychosocial assessment, advocacy, referrals, counselling and care plans

  • Evaluate the effectiveness of treatment plans and modify as required

  • Participate in care conferences and liaise with other health care professionals as needed

  • Maximize client treatment & clinical outcomes

  • Educate the general public and community workers in relation to our services and maintain waitlists

About you

  • Tertiary qualifications in Social Work and the relevant registration

  • Experience in the provision of social work services with adults/elderly favourable

  • Current First Aid & CPR certificate (mandatory)

  • Current Queensland Provisional or above (mandatory)
