Summary
The
Clinical Pharmacist
is responsible for delivering medication management services to medically and socially complex patients. Reporting to the Manager of Pharmacy Services, this operational role involves direct patient care, development of pharmacy protocols, interdisciplinary collaboration, staff training, and participation in quality improvement initiatives. The position requires strong clinical expertise, regulatory compliance, patient education, and involvement in team-based care planning.

Key Responsibilities

  • Develop medication protocols and ensure safe prescribing practices

  • Provide chronic disease management support (e.g., diabetes, COPD, CHF, asthma)

  • Participate in quality initiatives, patient interventions, and policy development

  • Educate patients and consult with healthcare professionals on drug therapies

  • Maintain compliance with pharmacy regulations and stay current on drug innovations

  • Support staff development and engage in team-based care coordination

Requirements

  • PharmD degree and active pharmacist license

  • Medicare Part B prescription coverage knowledge

  • 3+ years of pharmacy experience, preferably in clinical settings

  • Strong therapeutic knowledge, organizational skills, and tech proficiency

  • Effective communication, critical thinking, and ability to work independently and collaboratively

  • Licensure as a pharmacist in the state of Florida
