Catherine House Inc.
provides crisis, transitional and longer-term housing, and a range of services to women experiencing homelessness as part of the Housing Choices Group. The Intake Assessment, Diversion and Pathways Worker is a newly configured role established to support our evolving service model, including the Gateway Support Service and the planned Catherine House New Generation build. The role undertakes structured, risk-informed assessments of referrals and develops recommendations regarding suitability for Catherine House accommodation programs or other services, diversion, or alternative housing or support pathways, where Catherine House is not best placed to meet the client’s needs.

Skills and Experiences:

Key responsibilities include:

Processing Referrals

  • Conduct structured comprehensive assessments using agreed frameworks

  • Conduct assessment and make recommendations for one-off brokerage support to divert from a period of homelessness or resolve a financial crisis

  • Ensure a clear system for timely documentation of all referral information to ensure clarity for all staff to respond to if intake worker not available

Client Support

  • Identify and facilitate appropriate pathways or diversion options

  • Facilitate referrals to internal waitlist support and other Catherine House supports as appropriate, or to external services

  • Ensure mental health services are in place if required, and appropriate safety plan has been made available

  • Appropriately assess for family and domestic violence, and ensure a safety plan is constructed, where there is any risk to the client

Stakeholder Engagement

  • Build collaborative relationships with housing and accommodation providers, to ensure warm referrals are facilitated in a seamless manner

  • Work collaboratively with other workers involved in the client's support

  • Contribute to service improvement based on interactions with relevant stakeholders

Administrative Support

  • Be responsible for administrative tasks associated with the operational aspects of the program. These may include additional data management and reporting, client file management, or other related tasks as delegated by the line manager.

  • Provide trend analysis and reporting as required on clients’ support needs and unmet needs

  • Other tasks or duties as directed

You will bring

  • Qualified Social Worker eligible to be registered under the SA Social Work Registration Scheme OR

  • Relevant tertiary qualification and sufficient experience providing relevant services that enables eligibility for Provisional or Limited registration under the SA Social Work Registration Scheme

  • Minimum 2 years’ experience in assessment or intake roles within community or crisis services.

  • Experience working with women experiencing disadvantage, trauma, or mental health challenges.

  • Strong interpersonal skills with a compassionate, non-judgemental approach.

You must agree to undertake any required employment screening, which may include a National Criminal History or Police Check, Working with Children or Vulnerable Persons Check, and an NDIS Worker Screening Check.
