Firefly Children & Family Alliance is currently seeking a
Case Manager
in Indianapolis, IN. Our Case Managers collaborate with our internal team and community agencies to directly advocate for at-risk children and families by offering emergency shelter care, providing critical needs assessments, prevention and intervention services, and progress monitoring.

You Will
Client Progress - Monitoring and Coaching (50 %)

  • Coach Collaboratively-maintaining a balance between teaching and Coaching

  • Assist Families/Youth in obtaining their goals

  • Provide Education and support to you Respite Families/Youth

  • Provide Transportation to Families/Youth as needed

  • Communicate with schools and various institutions related to Families/Youth

  • Respond to all crisis calls when on call and manage/staff those situations.

Service Plans / Goal Development (20 %)

  • Request DCS case plans

  • Review and update plans and goals

  • Placement updates- communicate with FCMs on placement progress

  • Develop network of relationships with agencies in the community & participate in CFTM's (Child & Family Team meetings)

  • Work collaboratively with other social welfare agencies & Shelter NACS liaison

  • Refer when appropriate to community agencies based on needs

Intake and Needs Assessment (15 %)

  • Support Assistant Director with management of Intake Process

  • Complete Initial needs assessment and Determine Crisis Situations

  • Tier Support

  • CANS support

  • Ensure Data Tracking- RHYMIS/HMIS

  • Adhere to and follow Children's Bureau professional standards

Reporting (15 %)

  • Maintain case and agency paperwork according to standards

  • Prepare written reports for the Department of Child Services / Probation office/RHYMIS within identified timeframes

You Have

  • A bachelor's degree in social work, counseling, psychology, or related field is required.

  • Ability to travel between appointments in order to support the children and families in the community

  • A valid driver's license and insurance

  • Knowledgeable regarding social service systems within the state

  • Clear, effective communication skills

  • Knowledge of and adherence to current confidentiality practices

  • Appreciation for and ability to respond to cultural differences

  • Ability to develop and maintain relevant networks of key contacts

  • Adept prioritization and time management abilities

In terms of experience, we prefer that you have one to two years in child welfare or similar field.
